PERCOM Clinical Instructions, Forms, and Checklists
Students who have to do clinical rotations must have all of their clinical forms and documentation turned in 6 weeks prior to attending clinical rotations. Submissions made after the 6 week deadline may delay the start of clinical rotations. Paperwork may be sent in all at once or a little at a time. A check list is provided for each student to keep up with the paperwork requirements. All paperwork should be submitted to clinicals@percomonline.com. All items must be submitted to the Clinical Liaison even if you have previously submitted them to Admissions. This is to ensure that the Clinical Liaison has the lastes documents and to ensure they have a complete set.
EMT Basic Students should choose either PERCOM Corporate, Kilgore College, Texas Emergency Training (TET) or TEEX below (Click on the Pictures below for the requirements and forms for each site). Advanced students should choose Kilgore College or TEEX only. TET has clinical sites on the Western Side of Houston. Kilgore College has clinical sites in North East Texas. PERCOM Corporate and TEEX have clinical sites in various locations state wide. Please email the clinical coordinator to determine the best choice for you rotations if you are unsure which one to choose.
ALL Students are required to have a stethoscope and a watch with a second hand. Any student who does not have these items may be sent home from the clinical site. Any student who is sent home for not having these items will be considered a "no show" for that rotations and will be subject to the "no show" policy as stated in the Student Handbook"
