Clinical Practicum Requirements for TEEX
These are the Requirements for attending Clinical Rotations at TEEX. Below the requirements list you will find the links to download the checklist, forms and rules for attending clinical rotations. The check list is for you to keep track of the documents you will need to obtain and send in. The rules are for you to read before attending clinical rotations at the various sites. The forms must be printed off, filled out and sent back to clinicals@percomonline.com
- High School Diploma, GED or College Transcript.
- Shot Record (Must include a TB test within 6 months of the start of clinicals, Hepatitis B Vaccine, MMR (2 sets), meningitis, annual flu vaccine, tetanus and Chicken Pox (or proof of having contracted disease))
- 10 panel Urine Drug Screen (Within 30 days of the start of Clinicals. Student who will be attending clinicals at College Station Medical Center will have to take another Drug Screen at the clinical site.
- Criminal Background Check (contact clinicals@percomonline.com for details)
- Proof of Health Insurance (Short Term Insurance is acceptable)
- Copy of Drivers License
- Health Care Provider CPR Card (front and back)
- EMT Basic Card (If in advanced course)
- HIPPA Training Form
- TEEX Criminal Background Authorization
- Clinical Skills Acknowledgment
- TEEX Clinical Site Waiver Packet
- TEEX Registration Form
Rules
When filling out the Clinical Site Waivers, please print your name everywhere you sign your name. You do NOT have to get these forms Notarized.
